The trail officially starts near the main entrance to the hospital complex (Shoshana Biran Square), from where it ascends to its highest point (an elevation of 720 meters above sea level), then turns south towards Ein Sharig (its southernmost point). From Ein Sharig, the Hadassah Trail connects to the Springs Trail, descending down Wadi Yossef towards Ein Handak, its westernmost and lowest point (580 meters above sea level). Along Wadi Yossef, the trail passes west of the moshav Even Sapir, passing by ancient agricultural installations and watchtowers uncovered there, as well as the "small Western Wall" complex – an impressive agricultural site from the Herodian period featuring a dam or rather a retaining wall designed to stop soil erosion without holding back water; the dam is made of large kurkar stones that reminded hikers of the Western Wall.
From Ein Handak, the trail heads north along a Roman-Byzantine road until an elevation of 640 meters, then continues east towards Ein Kerem up to the eastern border of Guroni Monastery (the "Moscow", the Russian monastery) in Ein Kerem. The trail reconnects to Shoshana Biran Square as it passes inside the Hadassah Campus.
