Es ist nicht gesichert, ob Kaiser Heinrich II. das Land am Pyhrn bereits anfangs des 11. Jahrhunderts dem Bistum Bamberg schenkte. Jedenfalls gründete Bischof Otto II. von Bamberg 1190 das namensgebende „Hospital“ am Pyhrn, das er einer Laienbruderschaft anvertraute. 1418 wurde es in ein Kollegiatstift umgewandelt. Das Stift Spital am Pyhrn erlangte in den folgenden Jahrhunderten einigen Reichtum und wurde durch Papst Paul V. im Jahr 1605 zu einer Propstei erhoben. Vorübergehend tendierten die ansässigen Chorherren auch zum Protestantismus. Die Barockisierung der Kirche erfolgte von 1714 bis 1730 unter Johann Michael Prunner (mit Fresken von Bartolomeo Altomonte und Stuckarbeiten von Domenico Antonio Carlone). Der Dom verfügt auch über vier vorzügliche Altarbilder von Martin Johann Schmidt, genannt „Kremser Schmidt“, und zwei von Michelangel Unterberger.
Das Stift Spital am Pyhrn hat insofern eine besondere Bedeutung für St. Paul, weil nach seiner Auflösung 1807 zunächst Benediktinermönche des ebenfalls aufgehobenen Klosters St. Blasien im Schwarzwald mit dem gesamten Inventar an Kunstschätzen und Büchern sowie den Särgen der frühen Habsburger zunächst nach Spital umzogen. Da ihnen dieses aber zu klein und das Klima zu rauh war, übersiedelte der Konvent bereits 1809 nach St. Paul im Lavanttal. Die Stiftskirche wurde zur Pfarrkirche. Der stiftseigene Forstbetrieb wurde Grundlage für die spätere K.u.K. Forstverwaltung und der österreichischen Bundesforste.
Sehenswert ist heute das schmiedeeiserne Abschlussgitter von Andreas Ferdinand Lindermayr in der Stiftskirche, das zu den schönsten Schmiedearbeiten von Österreich zählt. Im Stiftsgebäude ist außerdem ein Felsbildermuseum untergebracht.
