Bereits am Ende des 12. Jahrhunderts entstand in der unmittelbaren Nähe des früheren Stadtzentrums der Oberstadt (des heutigen Baderplans) als Nachfolgerin einer bescheidenen Holzkirche eine romanische Kirche. Ihre mächtige zweitürmige Westturmfront ist bis heute erhalten geblieben.
Nach der Erhebung Grimmas 1220 zur Stadt wurde die Frauenkirche im gotischen Stil erweitert: das Kirchenschiff zeigt heute vier Spitzbogen-Arkaden auf quadratischen Pfeilern, die ihren Abschluss in einem Kreuzrippengewölbe finden. Die Querschiffe entstanden erst um 1462, nachdem ein Stadtbrand im Jahr 1430 eine Teilerneuerung der Kirche notwendig gemacht hatte. Bis zur Reformation unterstand die Frauenkirche dem Bischof von Merseburg und war Sitz eines Archidiakons.

Im 16. Jahrhundert wurde Grimma dann Sitz einer Superintendentur. In dieser Zeit gab es aus Scheu vor einer kostspieligen Instandsetzung der Kirche den Plan, die Frauenkirche abzutragen und ihre Steine für den Bau einer neuen Muldenbrücke zu verwenden. Glücklicherweise wurde dies durch die Intervention von Martin Luthers Freund Spalatin noch rechtzeitig verhindert. Im Inneren der Kirche ist der Taufstein in der Form der Spätrenaissance besonders sehenswert. Er wurde aus der im 13. Jahrhundert errichteten und 1888 abgebrochenen Nikolaikirche übernommen. Ebenfalls aus dieser Kirche stammen drei, um 1519 gefertigte, kunstvolle Altarfiguren: Petrus (im Altarraum links), Paulus (rechts) und Maria mit Kind (im Schiff links).

Der Altar

Das bedeutendste Kunstwerk der Frauenkirche aber ist der um 1510 entstandene spätgotische Flügelaltar. Der Altar gilt als Werk des „Meisters des Knauthainer Altars“. Er zeigt im Mittelteil die Geburt Christi, im Unterteil und den Flügeln weitere Szenen aus der Weihnachtsgeschichte (Maria und Elisabeth, die Anbetung der Weisen, die Verkündigung der Geburt Christi).

Die Glocken

Die Frauenkirche besitzt drei Glocken. Die große Glocke stammt aus dem Jahr 1453, die kleine ist nur zehn Jahre jünger. Die mittlere Glocke ging der Gemeinde im 2. Weltkrieg verloren. Sie wurde 1997 durch eine Neuanfertigung ersetzt.

Die Eule-Orgel

Hermann Eule aus Bautzen baute ein Vorgängerinstrument der Firma Jehmlich im Jahr 1928 grundlegend in der Frauenkirche Grimma um. Er versah sein Opus 167 mit einer pneumatischen Traktur und mit Taschenladen. Taschen sind die Lederteile, die die Ventile bewegen. Die mit Druckluft gesteuerte Orgel verfügt über rund 2.000 Pfeifen aus Metall, Holz und Leder.  Sie hat etwa 30 Register, verteilt auf zwei Manuale und das Pedal. Die Orgel wurde in den Jahren 1974 und 1995 erneuert. Im Jahr 2021 erfolgte die Fertigstellung der letzten Sanierung für insgesamt rund 80.000 Euro.
